Preferences Packet cover

Our custom Preferences Packet describes scenarios, medical treatments, and common comfort measures. The last section is dedicated to naming your medical power of attorney and having it witnessed. This makes the Preferences Packet a legal document.

A nurse will walk you through each scenario and option to ensure a full understanding.

We recommend having your medical power of attorney in attendance while you complete your Preferences Packet. Going through it together ensures they hear your wishes and your rationale. This helps if the day comes when they need to be your voice.
